Prepare for the season with four weeks of focused fundamentals, confident movement, teamwork, and fun. Boys and girls ages 5-8 of all skill levels are welcome.

Waiver and Consent

I, the undersigned parent or legal guardian, give permission for my child to participate in Nelson Basketball Academy Preseason Camp 2026.

I understand that basketball activities include movement, running, jumping, drills, and other activities that may involve risk of injury. I acknowledge that it is my responsibility to make sure my child is physically able to participate.

To the fullest extent allowed by law, I agree to release, waive, and hold harmless Nelson Basketball Academy, coaches, assistants, volunteers, staff, and related parties from claims, injuries, damages, or expenses that may arise from my child's participation, except in cases of gross negligence or intentional misconduct.

In the event of injury, illness, or emergency, I authorize camp staff to take reasonable action to assist my child, including contacting emergency services if necessary.

I have read and agree to the waiver and consent terms. I give permission for Nelson Basketball Academy to use camp photos or videos for academy communications and promotion.
